Taxpayer REQUIRED Documents


Before your appointment, please complete the REQUIRED questionnaire below.

Please review the list of typical documents needed in preparation for your appointment. Please make sure you bring all your tax documents to your appointment.

MARKETPLACE INSURANCE – If you have marketplace insurance for any time during 2025, you will need to include a form 1095-A in your tax return. If you do not have a copy of this form, you can obtain a copy at the link below.

IRS IPPIN – If you have an IRS issued PIN, you will need to have your 2026 IRS IPPIN for everyone who has an IPPIN on your tax return. If you do not have your 2026 IPPIN information, then you can request it here. Please note the IRS will not provide this information over the phone

If you are unable to print these forms, then please arrive 15-20 minutes prior to your scheduled appointment time to complete these forms. You can also come to the Food Bank location during our normal operating hours and pick up paper copies in the visitors entrance of the Food Bank. We are open on Tuesday, Thursday and Saturday from 9:15 am to 2:30 pm.
